Haya Albazzaz, PharmD

Haya Albazzaz, PharmD

Title:  Cardiology Care Specialty Resident

Haya received her Bachelor of Science in Biology and Pharmaceutical Science from the University of New Mexico. She also earned her Doctor of Pharmacy degree from the University of New Mexico, College of Pharmacy. Haya completed her PGY1 pharmacy practice residency at Banner University Medical Center in Phoenix, Arizona. Her professional interests include heart failure and mechanical circulatory support. Haya is excited to join the cardiology team at the University of Utah as a PGY2 pharmacy resident.

Outside of work, Haya enjoys running, cooking, and crafting.

Payson Ashmead, Pharmacist

Payson Ashmead, PharmD

Title: Health System Pharmacy Administration and Leadership Resident (second of two years)

Payson was born and raised in Ogden, Utah. After graduating from Brigham Young University with a BS in Molecular Biology and minors in management and German, he attended the University of Utah and received his PharmD. He is excited to continue pursuing his interests in medication safety, pharmacy operations, and drug policy as a second-year HSPAL resident.

In his free time, he loves spending time with his wife, Emily, and two sons, Owen (5) and Wes (3). He is elated to stay in Utah and continue skiing on the "Best Snow on Earth" and cheer on his hometown Utah Jazz.

Brittney Brito, Pharmacist

Brittney Brito, PharmD

Title:  Oncology Specialty Resident

Brittney is from the Atlanta, GA area but moved to Utah several years ago to attend Utah State University and pursue a degree in biochemistry. While working in a lab on campus that specialized in biomedical applications of synthetic spider silk protein, she discovered a passion for healthcare. Brittney then attended the University of Utah College of Pharmacy to pursue her career goals. Her areas of interest include ambulatory care, specialty pharmacy, and oncology.

Outside of pharmacy Brittney enjoys hiking, cheering on the Atlanta Braves and Salt Lake Bees, exploring new restaurants, traveling, paddle boarding, cooking/baking, and spending time with her husband, family/friends, and dog (Foxy the Pomeranian)!

Sara Ann Carr, PharmD

Sara Ann Carr, PharmD

Title:  Pharmacy Practice Resident (Acute Care)

Sara was born and raised in Bartonville, a small town in Central Illinois. She completed her undergraduate and PharmD coursework at the University of Iowa. She is excited to be completing her PGY1 Acute Care at the University of Utah Health and is looking forward to exploring a new state that is surrounded by mountains! Her clinical interests include oncology and infectious disease.

In her free time, Sara enjoys spending time outdoors, hiking, skiing, spending time with friends and family, playing volleyball, and cheering on the Iowa Hawkeyes!

Makenzie Daniels, PharmD

Title: Ambulatory Care Specialty Resident

I am from Brownsville, a very small town in Western Kentucky. I grew up surrounded by the profession of pharmacy. My parents met at the University of Kentucky College of Pharmacy where I went on to complete my PharmD curriculum. I completed my PGY1 at the University of Utah and am very excited to continue my growth at the U during my PGY2 in ambulatory care!

In my free time, I enjoy rock-climbing and traveling.

Clara Davis, PharmD

Title: Oncology Specialty Resident

Clara was born in Boise, ID and raised in International Falls, MN. She completed her PharmD at St. John’s University in New York City and decided to head back west to complete a PGY-1 residency at St. Luke’s Health System in Boise. She is excited to continue her training in Salt Lake City.

Outside of work, Clara can be found crocheting, exploring new restaurants, and cooking. She is also excited to go skiing and explore Utah’s natural beauty.

Erin Frohnapfel, PharmD

Title: Pharmacy Practice Resident (Acute Care)

Erin was born and raised in Alaska. She attended Gonzaga University prior to moving to Salt Lake City to attend Roseman University of Health Sciences for her Doctor of Pharmacy. She is ecstatic to be joining the University of Utah Health Acute Care team for her PGY1! Her interests lie in critical care and emergency medicine.

Outside of the hospital, Erin enjoys reading, kayaking, and spending time with her corgi (Sadie) and black labrador (Oakley).

Adriana Daniel Hudson, PharmD

Adriana Daniel Hudson, PharmD

Title: Pharmacy Practice Resident (Acute Care)

Adriana grew up in Massachusetts and went on to attend Carleton College in Northfield, Minnesota for her undergraduate degree. She earned her PharmD at the University of Minnesota and is eager to add Utah to her roster of states. Her professional interests include hematology and oncology and after completion of her PGY1 she hopes to pursue a PGY2 in oncology. She plans to continue her involvement in professional organizations throughout her residency.

In her free time, Adriana enjoys spending time with friends doing trivia and playing board games at local breweries. She also loves doing outdoor activities like hiking and paddleboarding and recently began learning to golf. She has two dogs and a significant other who are joining her in Utah and eager to become Utes!

Deni Hui, PharmD

Title: Oncology Specialty Resident

Deni was born and raised in Hong Kong, China. She moved to Texas when she was 14 to continue her education. She received her Bachelor of Science in Biochemistry and PharmD from the University of Texas at Austin. She completed her PGY1 pharmacy practice residency at the University of Utah and is excited to continue pursuing her interests in hematology and oncology as a PGY2 oncology specialty resident.

During her free time, Deni enjoys trying new restaurants, being outdoors, working out, traveling, and spending time with friends and family. She loves exploring the beauty of Utah including all the new hiking trails.

Gisoo Imani, PharmD

Gisoo Imani, PharmD

Title: Oncology Specialty Resident

Gisoo was born in Iran and raised in Salt Lake City, UT. She obtained her Bachelor of Science in psychology (class of 2019) and Doctor of Pharmacy (class of 2023) from the University of Utah. She completed her PGY1 Acute Care residency at the University of California Davis Medical Center in Sacramento, CA and is excited to move back home for her PGY2 in Oncology at the Huntsman Cancer Institute, where she had the opportunity to work with the most amazing preceptors throughout her internship and ultimately found her passion for this specialty.

During her free time, she loves trying out new brunch spots, taking daytrips to explore the surrounding nature, and looks forward to learning how to snowboard this year. She adds salt to pretty much everything and will heavily judge the salt content of every food she tries!

Jordan Kalthoff, PharmD

Title: Health System Pharmacy Administration and Leadership Resident (first of two years)

Jordan was born and raised near Colorado Springs, Colorado and completed her Bachelor of Science in Pharmaceutical Sciences and Doctor of Pharmacy from The Ohio State University (Go Bucks x2!!). She is so excited to be back near the mountains to grow her clinical and leadership skills at U of U Health. Her professional areas of interest include Operations and Systems Management, Pediatrics, and Investigational Drug Services.

Outside of pharmacy, Jordan loves to hike, snowboard, and travel. She can also often be found at concerts, at a workout class, or on a restaurant patio and is very happy to be doing her residency at a place that has it all!

Allyson Kluener, PharmD

Title: Solid Organ Transplant Specialty Resident

Ally was born and raised in Cincinnati, Ohio. She completed her undergraduate and PharmD coursework at the University of Cincinnati. Then she completed her PGY1 at the Christ Hospital Health Network (Cincinnati, OH). Ally is looking forward to joining us, and continuing her training as a PGY2 resident in solid organ transplant at the University of Utah Health.

In her free time, Ally enjoys trying new recipes, learning to make espresso, game nights, and doing anything outdoors!

Lauren Kobasuk, PharmD

Title: Pharmacy Practice Resident (Acute Care)

Lauren Kobasuk is from Columbus, Ohio and is excited to be in SLC and start her PGY1 Acute Care Setting residency. Lauren's interests are in emergency medicine, as well as critical care.

In her free time, she plans to hike, ski, and snowboard, go to concerts, and try out lots of new restaurants in the area.

Alexander Koo, PharmD

Title: Medication-Use Safety & Policy Specialty Resident 

Alex was born and raised in Las Vegas, Nevada. He received his BS in Pharmacology & Toxicology from the University of Wisconsin – Madison, MS in Pharmacology from Yale University, and his PharmD from Roseman University of Health Sciences in Henderson, Nevada. He completed his PGY1 Pharmacy Practice Residency (Ambulatory Care) with University of Utah Health and is excited to stay in Utah as a PGY2 Medication-Use Safety & Policy Specialty Resident. His clinical interests include drug information/medication safety, primary care, academia, and science/health literacy.

When not thinking about pharmacy, he likes to play boardgames, attend pub trivia nights, and develop his novice pickleball skills.

Title: Emergency Medicine Specialty Resident

Alexis was raised in Spokane, Washington where she also completed her undergraduate degree at Gonzaga University (Go Zags!). She completed her PharmD at Regis University in Denver, Colorado and most recently completed her PGY1 pharmacy practice residency at Banner University Medical Center in Tucson, AZ. She is excited to continue pursuing her interest in emergency medicine as a PGY2 here at University of Utah Health!

During her free time, Lexi loves hiking, being outdoors, baking, reading, and she almost always has a Diet Coke in hand. She is always up for an adventure and is excited to explore the beautiful area of SLC!

Chloe was born and raised in Fort Collins, CO and received her Doctor of Pharmacy from Creighton University in Omaha, NE. She is thrilled to be moving back to the mountains and joining the University of Utah HSPAL program. Her interests lie in transitions of care, pharmacy operations and medication safety.

Jayllex was born and raised in Dillonvale, Ohio. She completed her undergraduate degree at The Ohio State University (Go Bucks!) and her Doctor of Pharmacy at The Ohio State University College of Pharmacy. Her professional areas of interest include ambulatory care, endocrinology, and substance use disorders.

In her free time, Jayllex enjoys hiking, traveling, indoor cycling, attending concerts, and spending time with her dog, Oliver. She is looking forward to exploring all the natural beauty that Utah has to offer!

Title: Critical Care Specialty Resident

Chloe was born and raised in Boise, ID. She attended Boise State University (Go Broncos!) and obtained a degree in Mathematics. She moved to Salt Lake City, UT for pharmacy school and graduated from University of Utah College of Pharmacy with her Doctor of Pharmacy. She completed her acute care PGY1 at University of Utah Health. She is ecstatic to stay at the University of Utah for another year to complete a PGY2 in critical care.

Outside of pharmacy, Chloe enjoys playing soccer, pickleball, cooking/baking, and spending time with her husband and golden retriever (Dash).

Emi Radetich, PharmD

Title: Pharmacy Informatics Specialty Resident

Emi was born and raised in Salt Lake City, and evidently does not want to leave! She completed her undergraduate and PharmD studies and PGY-1 residency training at the University of Utah. Emi is ecstatic about staying on as a PGY-2 Pharmacy Informatics resident at University of Utah Health. In particular, she looks forward to exploring her current areas of interest in data analytics and artificial intelligence.

When not at the hospital, you can find Emi enjoying her time with family and friends, long-distance running and weight-lifting, and working in pharmacy professional organizations.

Title: Pharmacy Practice Resident (Ambulatory Care)

Joe was born and raised in Centerville, Utah. After graduating from Utah State University with a BS in human biology and a chemistry minor, he attended the University of Utah to receive his PharmD. He is interested in ambulatory care, emphasizing working with patient populations from minority groups. Joe is excited to continue his training as an ambulatory care PGY 1 resident with the University of Utah Health.

Joe enjoys rock climbing, hiking, mountain biking, snowboarding, and cooking in his free time.

Title: Health System Pharmacy Administration and Leadership Resident (second of two years)

Manisha is from Overland Park, Kansas and graduated with her BS in Pharmaceutical Studies and PharmD from the University of Kansas. She is excited to grow her leadership skills during her second year of residency. After completing her residency she hopes to be an inpatient operations manager.

In her free time, Manisha enjoys playing tennis, attending concerts and trying new restaurants. She hopes to see at least one of the many national parks in Utah before the end of residency!

Jackie was born and raised in Salt Lake City, UT. After completing her undergraduate degree in Biology, she continued on to obtain her Doctor of Pharmacy degree from the University of Utah. She completed her acute care PGY1 at the University of Utah Health. She is thrilled to stay at the University of Utah for another year (go Utes!) to complete her PGY2 in internal medicine.

Outside of pharmacy, Jackie enjoys traveling to new places, spending time with friends and family, and spoiling her golden retriever (Scout).
