Jared Baxa, PharmD

Jared Baxa, PharmD

Title:  Pharmacy Practice Resident (General Practice)

Jared was born and raised in central Minnesota where he completed his undergraduate education at St. John’s University. He went to the University of Wisconsin – Madison to complete his Pharm.D. degree. He is looking forward to continue his training through the completion of a PGY1 residency. His current professional interests include critical care, cardiology, and transitions of care.

In his spare time, Jared enjoys traveling, spending time outdoors, and watching sports (On Wisconsin!). He is excited to spend time hiking, biking, and skiing in the mountains.

Title:  Critical Care Specialty Resident

Azelia grew up in Miramar, FL and Bethel Town, Jamaica. She obtained her Bachelors of Science in Animal Biology from the University of Florida in 2010 (Go Gators!) and graduated from Mercer University College of Pharmacy in May 2017. She completed her first year of residency at WellStar Atlanta Medical Center and is excited to join the University of Utah team as a PGY2 Critical Care resident. In addition to critical care, Azelia has a special interest in cardiology, emergency medicine and neurocritical care and hopes to explore these topics and much more during her critical care residency.

In her free time, Azelia enjoys reading, writing, and trying new foods and recipes. She hopes to explore the local restaurants and the outdoor activities that Salt Lake City has to offer!

Title:  Solid Organ Transplant Specialty Resident

A Kansas City native, Bre earned both her Bachelor of Arts in human biology and Doctor of Pharmacy at The University of Kansas. She trained as a PGY1 Pharmacy Practice Resident at University of Utah Health and is looking forward to staying in the mountains to continue her training in solid organ transplant. In addition to transplant, her professional interests include internal medicine, patient education, and precepting.

In her spare time Bre enjoys spending time with friends and family, hiking, and watching live music. She also likes attending sporting events and will be cheering on the Kansas City Royals and Kansas Jayhawks basketball from afar!

Lillian Clark, PharmD, MBA

Title:  Health System Pharmacy Administration Specialty Resident (first of two years)

Lillian is originally from Virginia and earned an undergraduate degree in Biology from Virginia Tech. Afterwards she completed her PharmD and MBA at the University of Kentucky. Lillian is excited to begin working alongside pharmacy leaders and managers at the University of Utah.

Outside of work, Lillian enjoys trying new restaurants, exploring outside through hiking, and being with friends and family. In addition, Lillian is an avid sports fan and enjoys cheering on her favorite collegiate sports teams and Washington D.C. teams! She will spend the next two years in the program and is excited to see everything Utah has to offer.

Rebecca Clark, PharmD

Title:  Oncology Specialty Resident

Rebecca was born in Long Island, New York and grew up along the east coast in Melbourne, FL and Raleigh, NC. She completed her undergraduate studies in biology and earned her PharmD at Lipscomb University College of Pharmacy in Nashville, TN. Rebecca then moved to Detroit, MI where she completed her PGY-1 Pharmacy Practice Residency at Henry Ford Hospital. Her passion for hematology/oncology originated from her experiences in pharmacogenomic research and clinical rotations in pharmacy school. She is overjoyed and blessed by the opportunity to complete her PGY-2 training at Huntsman Cancer Institute and is looking forward to the coming year!

Outside of pharmacy, Rebecca enjoys spending time with family and friends, exploring the local food scene, attending concerts and festivals, and adventuring outdoors (especially skiing and hiking!)

Lynsi Collins, PharmD

Title:  Internal Medicine Specialty Resident

Lynsi is originally from Jacksonville, Florida. She attended the University of Florida for her undergraduate degree in microbiology before receiving her PharmD at the University of North Carolina. She then moved to Knoxville, Tennessee to complete a PGY1 pharmacy practice residency at the University of Tennessee Medical Center. She is excited to be joining the University of Utah team to continue her pharmacy career as one of the PGY2 residents in Internal Medicine. In addition to internal medicine, Lynsi’s professional interests include infectious disease and cardiology.

In her time off, she enjoys hiking, traveling, eating ice cream, and watching college sports. Go Gators! She is excited to explore the mountains of Utah and learn to ski!

Title:  Ambulatory Care Specialty Resident

Keaton was born in Logan, Utah, and completed his pharmacy degree at the University of Utah. He completed his PGY1 residency at the University of Utah with a focus in ambulatory care. He is excited to start his PGY2 Ambulatory Care Residency.

Outside of pharmacy, Keaton enjoys spending time with his family, camping, hiking, biking, and just about anything outdoors.

Title:  Pharmacy Practice Resident (Ambulatory Care)

A Philanthropist, scholar, and gentleman (self-designated), Jeremy DeLor comes to us from the cold streets of Detroit, where he simultaneously earned his doctorate of pharmacy while researching treatments for cancer* at the Wayne State University College of Pharmacy. After politely declining a starring role in The Avengers film series, Jeremy travelled to Salt Lake City to develop practice interests that include ambulatory care, infectious disease, and addressing health disparity.

When not fulfilling resident duties, which is essentially never, Jeremy spends his free time reading, hiding from his responsibilities in the mountains of Utah, and becoming overly emotional at the improper use of apostrophes and oxford commas.

*I actually did this, so it isn’t a blatant lie!

Louis Dunn, PharmD

Title:  Informatics Specialty Resident

Louis grew up in St. Paul, Minnesota and earned his PharmD from Creighton University in Omaha, Nebraska. He completed a PGY1 residency at Bethesda Hospital back home in St. Paul, where he developed a passion for informatics. Louis is extremely excited to join the informatics team at the University of Utah as a PGY2 resident.

In his free time, Louis enjoys playing basketball, biking, cooking, and spending time with friends and family (and dogs). Louis is also an avid sports fan, and is eager to explore Utah's beautiful parks and mountains.

Title:  Drug Information Specialty Resident

Rachael was born near Chicago Illinois, where she also attended the University of Illinois at Chicago College of Pharmacy. She completed her PGY1 with a focus in Ambulatory Care at the University of Utah health and is excited for the opportunity to continue her training here. Her practice interests include, drug information, HIV, and academia.

Outside of pharmacy, Rachael enjoys spending time with family, especially her three nephews and one niece. She also loves dogs and going for hikes around the Salt Lake City area. She hopes to continue to explore more of Utah in the coming months.

Camryn Froerer, PharmD

Title:  Health System Pharmacy Administration Specialty Resident (first of two years)

Camryn was born in Neenah Wisconsin but has lived in Utah since age 8. He obtained his bachelor’s degree in Biochemistry from Utah State University before attending pharmacy school at the University of Utah. His clinical areas of interest include emergency medicine, oncology, and postoperative care. As a first-year Health System Pharmacy Administration resident he is excited to explore ways to help make healthcare safer and more efficient for our patients.

Outside of pharmacy Camryn enjoys wake surfing, snowboarding, and watching the Jazz (in good and terrible years). However, his favorite thing to do is spend time with his wife and their dog.

Mika Jankowski, PharmD

Title:  Oncology Specialty Resident

Mika Jankowski was born and raised in Phoenix, Arizona. She completed her bachelors’ degrees in molecular and cellular biology, physiology, and creative writing with a minor in chemistry, as well as her PharmD at the University of Arizona (Go Wildcats!). After spending most of her life in the desert, she was excited to move closer to the Rocky Mountains and complete her PGY1 pharmacy residency at the University of Colorado Hospital in Aurora, Colorado. Mika is thrilled and honored to start her oncology career at the Huntsman Cancer Institute, where she will have a variety of hematology/oncology experiences, as well as teaching opportunities. In her free time, Mika enjoys traveling, cooking, exploring new restaurants, hiking, and spending time with her 10 pound Boston terrier. She is looking forward to exploring the national parks in Utah this year, as well as the ski slopes in the winter!

Title:  Pharmacy Practice Resident (General Practice)

I come from Valsad, a small town located on the west side of India. A personal tragedy related to an adverse drug event was vital in shaping my career path. It instilled in me an interest in the crucial role of a pharmacist in saving patients’ lives. I completed a Bachelor of Pharmacy in India back in 2009 and moved to the United States where I continued my pursuit of becoming a pharmacist. I completed Doctor of Pharmacy from The Ohio State University College of Pharmacy. During pharmacy school, I gained experience in medication safety through an internship at the James Cancer Hospital. I strive to complete a PGY2 Medication-Use Safety Residency or a residency with a medication safety component in it. I want to make healthcare a safer place for both patients and caregivers while advocating for the profession of pharmacy. I am very thankful to my mentors, family, and friends for their incredible support and inspiration.

In my free time, I like to listen to Indian music, watch news/movies, hike, run, cook, and hangout with friends and family!

Elizabeth was born and raised in small town Dodge Center, Minnesota. She earned her PharmD from Ohio Northern University in Ada, Ohio. She is looking forward to continuing her training as a PGY1 resident at University of Utah Health and exploring her professional interest areas of oncology and critical care.

Outside of pharmacy, Elizabeth enjoys spending time with friends and family, attempting to cook, hiking, and traveling. She especially enjoys exploring the outdoors and is excited to see some of the sights Utah has to offer!

Joanne was born in the suburbs of Chicago, IL where she completed her undergraduate degree in biological sciences at the University of Illinois at Chicago before earning a PharmD at the University of Wisconsin-Madison (go Badgers!). She is eager to start residency in Salt Lake City to explore her different professional interests, which include oncology and transitions of care.

In her spare time, Joanne likes to work out, try new recipes, cheer on the Chicago Blackhawks, and is excited to get a little lost in Utah’s parks.

Title:  Health System Pharmacy Administration Specialty Resident (second of two years)

I was born and raised a Buckeye in Akron, Ohio. I completed my B.S. in Pharmaceutical Sciences and Doctor of Pharmacy at The Ohio State University. After pharmacy school, I travelled to Salt Lake City to complete my PGY1 at University of Utah.

In my spare time, I enjoy spending time outdoors with my husband and our pug. I am an avid sports fan and especially enjoy baseball and football (Go Bucks!). I have thoroughly enjoyed exploring Utah over the last year and am excited to continue that journey.

Rebecca was raised in Saratoga, New York and completed her undergraduate studies and PharmD degree at the University at Buffalo School of Pharmacy and Pharmaceutical Sciences in Buffalo, New York. She went on to complete her PGY1 training at WVU Medicine at Ruby Memorial Hospital in Morgantown, WV. She is excited and honored to continue her training in oncology at the Huntsman Cancer Institute.

In her free time Rebecca enjoys spending time outdoors, hiking and running. She is excited to ski out west, visit national parks and explore all that Salt Lake City has to offer!

Rebecca was born and raised in Indianapolis, IN and earned her PharmD at Purdue University. She completed her PGY1 residency training at the University of Toledo Medical Center, during which she developed interests in hepatology, anticoagulation, and patient education. She is very excited to continue her training in Utah as one of the PGY2 internal medicine residents!

Rebecca is a loyal Colts/Pacers/Boilers fan and enjoys running, watching documentaries, and trying new recipes in her free time.

Title:  Pharmacy Practice Resident (Ambulatory Care)

Nattie is from a small, picturesque town in the Colorado Mountains. She completed both her Bachelor of Science in Chemistry and Doctor of Pharmacy at the University of Utah, and she is thrilled for the opportunity to continue her training with University of Utah Health. Nattie is passionate about optimizing patient care through the practice of pharmacy within collaborative interdisciplinary teams. She is especially interested in primary care, women’s health, cardiology, and drug information.

Outside of pharmacy, Nattie has remained connected to her roots and loves spending time in the mountains. She enjoys being with family, all forms of exercise, and anything that involves being outdoors.

Title:  Health System Pharmacy Administration Specialty Resident (second of two years)

I was born in Sacramento, California and raised just outside the Twin Cities in Minnesota. I completed my PharmD and MBA at Drake University in Des Moines, Iowa. I have had a great experience during the first year of the HSPA residency. As I move into the second year my primary interest areas include pharmacy informatics, clinical analytics, and medication safety.

In my spare time, I am an avid runner, cyclist, and triathlete. Since moving to Utah I have enjoyed taking advantage of the numerous hiking and biking trails and snowboarding in the fresh mountain powder!

Title:  Pharmacy Practice Resident (General Practice)

Meredith was born in Laguna Hills, California and grew up in a military family with the privilege of living coast to coast. While attending the University of Mississippi, she graduated with a BS in Pharmaceutical Sciences and a diploma from the Sally McDonnell Barksdale Honors College in 2015, and her PharmD in 2018. Meredith’s pharmacy interests include infectious disease, academia, and health policy. She is overjoyed to be joining the University of Utah team for her PGY-1 residency and has a goal to pursue a PGY-2.

Outside of pharmacy, Meredith enjoys traveling, painting and practicing yoga. She is looking forward to bringing her mountain bike with her to Salt Lake City while exploring all the natural beauty and outdoor recreation Utah has to offer.

Sabrina was raised in Boise, Idaho. She attended Idaho State University for both her undergraduate and pharmacy doctorate studies. Growing up in a state without a large medical center, Sabrina quickly learned that University of Utah is where Idaho residents go when they need specialized care. She feels fortunate to have had the opportunity to serve patients at University of Utah Health as a PGY1 Pharmacy Resident. She is excited to be continuing her training at our institution as a PGY2 in internal medicine. Sabrina's specific clinical interests include gastrointestinal and pulmonary disorders.

Sabrina enjoys hiking, finding new coffee shops, and being a photographer.

Title:  Infectious Disease Specialty Resident

Brandon grew up in Valparaiso, IN. For undergraduate studies, he attended Wabash College, earning a degree in Biology. Later, he earned his PharmD from the University of Wyoming and completed a PGY1 Pharmacy residency at Intermountain Medical Center. He is excited to continue his pharmacy training as an Infectious Disease Pharmacy Resident at University of Utah Health. His areas of professional interest include antimicrobial resistance mechanisms and antimicrobial pharmacokinetics/dynamics.

Besides pharmacy, Brandon enjoys rugby, whitewater rafting, reading, puns, and a never-ending Netflix cue.

Originally from Kentucky, Kelsey received her Bachelor of Science degree in Chemistry from the University of Kentucky and her Doctor of Pharmacy degree from Union University College of Pharmacy in Jackson, Tennessee. Kelsey completed her PGY1 training at St Joseph East in Lexington, Kentucky and is looking forward to completing her PGY2 training in oncology at the Huntsman Cancer Institute.

In her free time, Kelsey enjoys kayaking, playing the cello, and trying new restaurants around town.

I was born and raised in Smith Center, Kansas before attending Pittsburg State University as a student athlete to complete my pre-pharmacy requirements. I then earned my PharmD from the University of Kansas in Lawrence, Kansas. I am excited for the move to Utah and to begin training for a career in ambulatory care pharmacy.

Most of my free time is spent following my favorite local sports teams, the Jayhawks, Chiefs, and Royals. I am eager to take full advantage of all the outdoor activities Utah has to offer!

Chenshan Zhou is originally from Kansas City and earned her pharmacy degree from The University of Kansas. Rock Chalk! She completed her PGY1 pharmacy residency at Saint Luke’s Health System in Kansas City. She is excited to join the University of Utah Hospital to continue her pharmacy career as one of the PGY2 residents in Critical Care.

Outside of pharmacy, she enjoys spending time with friends and family, traveling, and sampling new cuisine. She is looking forward to exploring all that Utah has to offer.
