Insulin Syringe (ReliOn) Recall

Covidien (Tyco Healthcare), the manufacturer of ReliOn insulin syringes, are recalling a single lot (813900) which was distributed between August and early October, 2008. The syringes are for use with 100 unit insulin and are mislabeled. The mislabeling could result in an overdose of as much as 2.5 times the desired dose resulting in severe adverse effects. Advise patients who use these syringes to double check the lot number on either the carton or the individual syringe package.
